Vol.LXVII, No.22 Thursday, June 1, 1995 We Say Are health-care costs spiralling?ne of the traps residents can fall into O: they are forced to defend the existence of their local hospitals in consultation meetings later this month, is the notion that health-care costs are \u201cspiralling,\u201d as some politicians and commentators would have us believe.It\u2019s as if the phrase \u201cspiralling health-care costs\u201d has been on the tip of every tongue that has wagged in front of a microphone the past two or three years, be it the tongue of a journalist or a politician, so that now it passes as the unquestioned truth.Once it is the truth, the government is justified in taking a scalpel to our health-care system.\u201cDo what you must, Doctor,\u201d says the public, hiding their eyes.\u201cBut don\u2019t make me look.I can\u2019t stand the sight of blood.\u201d And with our permission, the Health Minister performs radical surgery on our health care system as his colleagues in the other ministries \u2014 aspiring budget-surgery interns one and all \u2014 watch from the gallery.The public shrugs sadly and says, \u201cIt had to be done.After all, costs were spiralling.\u201d But are health-care costs really \u201cspiralling\u201d?They say it\u2019s easy to lie with statistics.But take a look at the amount being spent on health care, compared to the total being spent each year by the province and you'll wonder who's telling the truth.In 1974-75 \u2014 when Medicare was fully implemented, current hospitals were in place, and CLSCs did not yet exist \u2014 the Quebec government spent $1.16 billion on health care out of a total budget of $6.51 billion or about 19.2 per cent of the total provincial budget.A lot of money, it\u2019s true.Lavy Making right choices about hospitals Since news was released about the closure of several Montreal hospitals there has been a great deal of public outcry and demonstration concerning the fairness of the decision of the Quebec government.Few people disagree that cutbacks must be made somewhere to try to reduce Quebec's enormous debt but most people feel that the seemingly arbitrary closure of several hospitals in the Montreal area is not the way to go.What is very frustrating about the present situation is the fact that the government id - It reflected our priorities \u2014 how much we valued our emerging health-care system, which offered quality care for all, regardless of the ability to pay.Fast forward 20 years to 1994-95, when the government spent $9.96 billion on health care out of a total budget of $42.13 billion or about 23.6 per cent of the budget.So health care costs have gone up \u2014 from 19.2 per cent to 23.6 per cent, or about 4 per cent of the total budget in 20 years.Are they \u201cspiralling out of control\u201d?No, clearly they are not, at least when viewed as a percentage of total spending.In other words, spending on health care has gone from about one-fifth of the budget to between a fifth and a quarter.An increase, certainly.But certainly not rising uncontrollably, exponentially, out of control, or in any other way some would have you believe.Of course, spending has to be reduced.The government has to stop spending money it doesn\u2019t have.But don\u2019t let them tell you that health cuts are a must because costs have gone through the roof.It is spending on non-essential services and political pet projects that has gone through the roof.If we're really down to the crunch, then that\u2019s where the budget axe should fall first, not in health care, which should remain a top priority among services.Meanwhile, nobody should be able to get away with using a slash and burn approach, closing a string of hospitals to save $190 million on a total health budget of more than $9 billion.It doesn't add up.\u2014 Bernie ONeill have an alternative.Why not decide that in a certain period of time, say six months, the government must close down a certain number of hospitals?In the next six months hospitals would compete as businesses do.Each one would do its best to cut waste, improve its organization, and provide the best possible health care.Everyone would benefit.At the end of the time period the 10 least efficient hospitals would be closed down, and the remaining hospitals would continue to provide their optimal service in order to remain competitive.Sarah O\u2019Brien ; SL.ns WESTMOUNT, Let\u2019s retrain our cities \u2014 Part Ill get my hair cut only when it becomes Jue necessary.For me, undergoing tonsorial trimmings is not a shear delight.This is not meant to reflect badly on the obvious charms of Rose, the delightful lady who cuts my hair.She is the barber of civility.(Should she be called a coiffeuse?A hairdresser?Or a trichologist, perhaps?) Anyway, Rose told me (snip, snip) how she recently moved from St.Leonard to Anjou (snip, snip).And how she doesn\u2019t mind the added travelling distance to her place of work near Westmount Square.By car.I asked her why she no longer took the metro.The only reason | could winkle out of her had something to do with her aversion to metro crowds \u2014 having to mix with the great unwashed (or uncoiffed?).She also mentioned the metros unreliability.But essentially, her decision turned on the splendid moving isolation afforded by a car.Thousands of Roses help explain public transit\u2019s steady drop in ridership.The privacy of the car guarantees its primacy as the mode of transport of choice for 60 per cent of regional travellers.Only 25 per cent use public transit.We should not be surprised, then, to learn that 80 per cent of commuting cars have only one occupant.A car becomes a mobile personal space.Considering this clear preference for cars, why do we spend $0.9 billion per year on the MUCTC, only 36 per cent of which is paid by passengers?In the Greater Montreal Region, governments pay $0.7 billion toward mass transit costs.They spend a similar amount (S0.8 Our Mayor Peter F Trent says.billion) for road maintenance and construction \u2014 for more than twice as many commuters.What's wrong with this picture?Well, it doesn\u2019t include the $10 billion car owners in the region spend each year for their cars, and the value of the vast amount of public land given over to roads and autoroutes.We may never be able to calculate the true cost of the car to society, especially the cost of pollution; but, unfortunately, it is all easy to reckon the cost of public transit.One thing is for sure, our roads are choked.We need four new bridges.If we added 12 lanes to the Metropolitan Boulevard, it would be full the next day.There are only two solutions: either get people to work on off-peak hours, or extend the metro and the suburban train network.Since Quebec produces few cars and no oil, cars only have a 45 per cent Quebec content.À train ticket represents 80 per cent, a metro ticket 95 per cent.But our metro, of the same petite dimensions of the 1898 Paris subway it was modelled after, and a product of a now-aban- doned rubber-tired '60s Parisian experiment, cannot be used overground.And, except for an extension to Laval, population density doesn\u2019t justify metro expansions.So the only way out of total traffic chaos in the next decade is to invest in surface rapid transit: suburban trains.Financed by taxes on gasoline or electronic tolls.Or by a private venture.But not by property taxes.Our MP Lucienne Robillard says.Baptism by fire often refer to the period occurring [on after my appointment to the Federal Cabinet as Minister of Labour as my \u201cbaptism by fire.\u201d I had certainly not expected to be faced with a national railway strike and work stoppages at both the Ports of Montreal and Vancouver within weeks of assuming office.My initiation to the Labour portfolio was intense and highly instructive \u2014 [ am certainly much wiser for the experience.As | have stated in the House of Commons, I do not like asking Parliament to intervene in labour disputes.| believe that responsibility for achieving negotiated settlements must rest with the parties involved.I also believe that there must be an effective process in place which enables labour-management disputes to be resolved quickly and equitably.To this end, my officials, in consultation with business and labour, are currently conducting a comprehensive review of the Canada Labour Code.Although collective bargaining in the federal jurisdiction has generally served us well, 1 believe that our current arrangements could be more effective in addressing labour disputes.The review of the!
